Wellness questionnaire
A short daily self-report — sleep, stress, mood, soreness, fatigue — that powers the team's readiness picture.
A one-minute morning self-report — how you slept and feel, plus a note to the coach.
On its own each entry is small. Across a squad and across weeks it becomes the team's readiness picture, turning subjective state into trended data before the numbers from devices catch up.
Fill it in
Athletes step through the questionnaire as a short wizard — one question per step, with large tappable options and Back / Continue to move between them.
From your Training Log, open the day and choose wellness questionnaire from the day card's ⋯ (more) menu.
The first step is the date the record is for — it defaults to today.
Step through each dimension in turn — in this instance Sleep, Stress, Mood, Soreness, then Fatigue. Each step shows a plain-language question and a row of options, each a number and a label (for example 1 Excellent … 5 Very bad sleep). Tap your answer and Continue.
The final step is a free-text note to the coach — use it when something's off ("slept badly, travel") so it gets read alongside the numbers.
Submitting saves the day's record. The questionnaire is not auto-saved — closing it part-way prompts to confirm, and the record is created only when you finish.
One record per day
There's a single record per day per athlete. Re-opening the same day edits that record rather than adding a second one.
Each record is kept in a per-athlete table — one row per day, with a column for each dimension (here Date · Sleep · Stress · Mood · Soreness · Fatigue · Notes) and every header filterable. From the table you can edit or delete any day's record.
For coaches
Select an athlete from the group panel to read their records. A coach can also fill a record on an athlete's behalf — New record opens the same wizard — and edit or delete any row from the athlete's table.
For the group view, head to Analytics: the Wellness analysis trends each metric over four weeks, and Team daily readiness combines the day's self-report with device recovery data and medical status for the morning overview.
How it connects
- The day's self-report feeds the Wellness analysis and Team daily readiness — the single morning "who's ready" view.
- It complements the same day's Training Log, which is also where athletes reach the questionnaire.
- It sits alongside the other readiness inputs — device recovery from Integrations and training-limitation status from the Medical module.
